• Full name of person whose record is requested
• Sex
• Parents names (maiden name of mother)
• Month, day and year of birth or death
• Place of birth or death
• Relationship to person whose record is requested

The Utah State Legislature recently passed a law to make historical death certificates more available to genealogists. Death certificates now become public 50 years after death.

There are only a handful of records earlier than 1900.
